If I recall correctly, if your agility is shit, you won't hit anything. Alsp, bottoming out your stamina (which you can easily do by running) will also majorly impact your hit chance.

Basically what factors into successful hit rolls is how full your stamina is, your skill in the weapon you are using, and your agility level overall. I can be a god in longsword and still miss a lot of hits if my stamina is depleted and my agility is low or average.
